D- According to the patient, she is stable at her current dose. She appears to be surprised about her dose. The patient had thought she was receiving 39mgs, not aware about her decrease to 33mgs. The patient then informed this writer that she had completed the dose change request form and spoke with Nursing regarding to the decrease and is pleased that she had actually gotten her request.…
I was confused because even the primary nurse was concerned the patient was abusing the drug. Still, even when the nurse asked what she could do to prevent this, the response from the doctor was “nothing,” because we could not prove the patient was abusing the drug. This makes me wonder if we should even be using the pill box and just come every day to administer the medication. Another issue is the patient can get more of the medication if they are in need of it. Medicare will pay for additional medication while being a hospice patient.…
When I was preparing the summary care plans for patient one and two I learnt about how to search for evidence and how to formulate a care plan based on the evidence obtained. This is particularly important to provide patient-centered care. I could clinically check prescriptions and if there were any interventions then I would refer to the pharmacist in charge.…

In order to avoid any chance of having consumers given the wrong prescription is to just have patients simple ask about what that certain drug is and how is can effect their body in particular because, their doctor knows all about their body already. Asking is always better then almost demanding for the product, also if the doctor will not explain what the medication can do the pharmacist that is giving the patient their prescription can also explain what the medication will and can do to the patients’ body. This then allows a more honest conversation with the doctor and allows both the patient and the doctor to come to an agreement on the what the course of action that needs to be taken to ensure that the patient is well taken care of and…

Statistics also state that nearly 1% of hospital admissions have an adverse drug event (Runciman, Roughead, Semple, & Adams, 2003)As a graduate nurse looking to continually improve and develop my practice I need to develop strategies that will help me progress towards this goal. This goal will help to prevent some of those adverse drug event and prevent some of the medication admissions entirely. Being in a position that is directly responsible and accountable for the administration of the medications in a lot of the cases means that I need to continually strive to improve and maintain a high level of medication management. I have already mentioned a few strategies that I believe will help me to continually develop those skills to a desired level. familiarising myself with the medication will help to reduce the risk of a medication error slipping through and effecting the patient.…
Some medication comes in both liquid and tablet form and it is important to select the appropriate form for your patient if it is not stated. Through this progression, it was important that I observed staff nurses and the methods in which they implemented in order to improve my own practice; each staff nurse I observed had a different method of practice for medication rounds, providing tips in order to improve my practice and the promoting of patient welfare for the medication rounds. Whilst I was dispensing medication, I ensured to ask the nurse accompanying me to check that I had calculated and measured the medication properly, providing positive encouragement and criticism where…

Safety Process: Six Rights and Three Checks The administration of medication is an integral component of the healthcare provision in the modern world (Koehler & Schwinghammer, 2008). The observance of the Six Rights and Three Checks principles are an example of measures that are undertaken to guarantee safety and positive outcomes in regard to the administration of medication (Koehler & Schwinghammer, 2008). The Six Rights in drug administration should revolve around the correct person receiving the medication, the nature of medicine,…
There is not one population that would benefit since a majority of patients feel the need to put responsibility on the caregiver. Patients many times leave out allergies, concern and other questions with fear they are wrong. This brochure covered five areas allowing patients at risk to feel comfortable in asking questions. Not only were example questions outlined, it created key points for the patient to observe during their care. The practice avoiding medication errors can now be gradually increased with the partnership of patient and caregiver.…
