The sodium level during this time was 131 mEq/L. The mother verbalized that the patient had not been using the bathroom as much as she usually does. The child’s BUN was elevated, which also can be due to dehydration or the medications she was currently on. My goal for this client was to have normal vital signs by the time I left the hospital that day. At 0800 that morning, the patient was tachypneic and had an elevated blood pressure. During my 1100 rounds, the patient’s vitals had not yet improved, so my goal had not been met. My last diagnosis for this client was impaired tissue integrity. During this admission, the patient had acquired diaper rash. This was more than likely due to her Vancomycin and Ceftriaxone antibiotics that were being given via IV. Nystatin cream was being used every six hours to help heal the rash. Teaching the parents hygiene is very important to prevent more rashes from appearing. Keeping the child’s diaper changed and maintaining adequate nutrition are very important factors in ensuring the patient will heal while preventing other rashes in the future. The best way to diagnose pneumonia would be a sputum culture. Arterial blood gasses should also be drawn to determine if the client is respiratory acidosis or alkalosis. A complete blood count should also be done to determine the amount of white blood cells. Chest x-rays are also an option, but they are usually done in severe cases. Chest x-ray can also be used when the antibiotics are not working, or when a patient is already hypoxic and in respiratory distress (Schauner et al., 2013). My patient was in respiratory alkalosis because she had a low carbon dioxide level. Her white blood cells were ten; this does fall within the normal limits of white blood cell counts. She also had a chest x-ray done; the doctors did posteroanterior and lateral to view which part of the lobes were effected. The lower lobes of both lungs had both been
