While performing an assessment on a vaginal primipara, I noticed that she started crying. I started to assess the reasons why. She reported that she was exhausted and had not slept in four nights, her nipples hurt and breastfeeding was not going as planned. Her newborns bilirubin levels were a little on the high side and needed formula supplementation to speed up excretion. I explained to her that her feeling were completely valid, that she had planned and pictured a beautiful experience with her newborn and reality was not meeting her expectations. This week I read an article stating that research shows that starting a general diet earlier helps kick start GI motility. The article was brought to my attention by a resident. After reading the article and looking at the evidence, I learned that early implementation of a general diet (within a couple of hours) does not compromise safety or comfort. Early feedings in postcaesarean women coralign with increased patient satisfaction and decreased perception of pain. The new research was implemented by half of the nurse on the unit by the next day.
