If I were the communication advisor to Tina, I think, several ethical communication issues from Tina's perspective are there:
First, she should listen to more staffs in different levels about the reasons of referrals dwindled, at least she should check it after the two middle managers told her. Since she wants to be a leader who is more involved, making sure to get a whole scope of accurate information is necessary for right decision making.

Firing the two PT directly without efficient communication with employees makes them more worried.
Some recommendations may work:
1. A deep meeting to communicate with all staffs is imperative. Encourage Jan's in-time report, admit own rough decision-making, and ask for suggestions. This will show Tina as a responsible and broad-mind leader and improve staffs to express their mind to solve recent problem.
2. A reasonable explanation for her strategy to give more confidence to the employees.
3. Tina'd better highlight that recent adjustment is to help the poor sustainably.
4. Ask everyone to get together to get through the difficulty and encourage Lisa and Claire to do their best.
5. Efficient communication with hospitals.
I think, to run a company well, different roles should explicitly be aware of their o n duty and the importance of finding, analyzing, communicating, solving potential problems. Jan, as an intake nurse, has done her job well and reported the PT problem in time to the right person. Jackie_initial
