'To Err is Human', that quote explains that humans are prone to errors. Our goal in healthcare organization is to minimize the chances of these errors.
Patient safety can be defined as ' the prevention of harm to patients' or ' is the absence of preventable harm to a patient during the process of health care'.
Upon bringing up the topic of patient safety, then we’re talking about how hospitals and other health care organizations protect their patients from errors, injuries, accidents, and infections. In the medical field, some hospitals are able to keep their patients safe while other hospitals can't. 440,000 people die every year from preventable errors in hospitals. About 20,000 people in the U.S. and 5,000 in the U.K. die …show more content…
The whole healthcare organization including healthcare professionals and patients are committed to build a safety culture.
The complexity of the patients is increasing. Patients are becoming with multiple co-morbidities. The chance of deterioration of these patients is very high and for many reasons. It is the hospital responsibility to provide a system through which it can provide maximum patient safety and care. The right care at the right time is the key to decreasing the rate of mortalities.
Patient safety is a shared responsibility to all the healthcare providers.
When we talk about patient safety, we have to take into consideration the following: • Safety – is all about avoiding and preventing harm to patients
• Effective – Evidence based service and treatment provided to the patient.
• Patient-centered – The patient needs and requirement is the center of the patient care.
• Timely – Promote action may prevent many harmful side effects to take place.
• Efficient – Avoid wastage of resources, energy and ideas. • Equitable – The quality of care provided shall be standard regardless any personal characteristics such as gender, geographic location, and socioeconomic status. …show more content…
The introduction of Medical Emergency Team – MET will be responsible in delivering the appropriate level of care.
Patient safety indicators:
Patient safety indicators are a group of indicators that help the hospital to measure the adverse incidents and complications that the patient experienced throughout his stay in the healthcare organization.
Patient safety indicators that we must take into consideration upon discussing the introduction of MET will be:
1. Failure to rescue from complication rather than the primary diagnosis
2. Cardiopulmonary arrests that take place outside the intensive care unit – ICU, represent failure-to-rescue events.
World wide, many hospitals are creating 'Rapid response team or Medical emergency team' – MET.
• At university of Pittsburgh Medical center Presbyterian Hospital in the US, the MET was implemented. After implementation of MET a 17% decrease in the incidence of cardiopulmonary arrests was noticed.
• In Baptist Memorial Hospital in Memephis, upon implementing the MET a 26% drop in cardiac arrests was experienced. Not only this but also the survival rate of cardiac arrests, improved greatly from 13% to 24%. The mortality rate was decreased by
