As discussed by Healthy People 2020 (2014), the broad category, social determinants of health include but is not limited to the following: the conditions in which people are born, exposure to crime, social norms, education opportunities, transportation options, public safety, exposure to mass media, job opportunities and social interaction, which all have an effect on an individual’s health and vitality. According to Centers for Disease Control (2018), social determinants of health such as poverty, unequal access to health care, lack of education, stigma, and racism are underlying, contributing factors of heath inequities.

Once the chain of infection has been identified, it’s very important for the community nurse to be able to break the chain of infection by providing education to all those involved including the community. Getting people to understand the proper ways to prevent spreading infection and bacteria includes frequent hand washing, good hygiene, proper food storage and keeping things clean in their living environment. Depending on the type of infection, there are other ways for the community nurse to break the chain. It’s very important for the community nurse to be equipped with personal protective equipment in order to prevent splatters into any open areas such as broken skin respiratory tract, and mucous
