It is vital that the team help educate the patient with medication, how to use instruments that they are exposed to, and therapeutic skills that they can do once they are discharged. It is essential that teams observe the patient and let them demonstrate what they’ve been taught. Breath sounds are normally clear or scattered fine crackles at bases, which clear with deep breathing. The presence of coarse crackles during late respiration indicates an airway obstruction, teams should help educate each other on the patient's condition and what to expect to care for the patient more effectively. The semi fowlers position, with the head of the bed elevated 45 degrees, allows the diaphragm to expand. The incencitive spirometer helps expand the lungs, and help prevent other complications such a punomonia. Alternative therapies such as guided imagery, and altering environment may be helpful in increasing patient comfort. The patient saturation before the change of shift was at 91%. This short term goal was achievable due to clear communication skills the team. The incoming nurses were able to maintain the patient oxygen saturation of at least 90%. The patients pain was well controlled and decreased dreastically within the 12 hour shift. In addition to medication, the respiratory team was able to teach the patient effective ways of using instruments to improve adequate breathing. Patient showed signs of increased comfort by being more verbal to the staff, and showed curiousity in his treatment process. The patient long term goal of remaining free of adventious lung sounds during a hospital stay was not achieved. The patient had multiple episodes, of course crackles during hospital stay. The patient will be transferred to a nursing facility where he will receive around the clock care. Although patients long term goal was not achieved, the nurses will obtain their roles in providing care for the patient. The team will work together in monitoring the patient, controlling patient pain scores, administer medications as orders following the six rights, enforce breathing skills, and auscultating sites for abnormal
