On this date worker received a call from Ms. Dana Long, Amedisys Hospice, for the purpose of following up on Mrs. Bertha Guthrie. Ms. Long stated they was providing services following her rehab days. According to Ms. Long, they have two concerns, Ms. Guthrie not receiving her meds during the day when Ms. Phyllis Guthrie is at work and she was wet during home visit.
