Questionnaire design is critical research activities. The researcher should consider the question that will be used to gather data from respondents, if the right question is not given to respondents, the obtained data is useless. The questionnaire must be a well- organized, the question must be clear, relate directly to research purpose and there is a nature order in order for respondents to complete the questionnaire properly Hence, the questionnaire must be designed in a specific way (Lee 2005, p. 761; Greener 2008, p. 10).
For this study, the questionnaire was designed base on the Theory of Planned Behaviour model combines with other variables by adopting close ended questions to collect data from respondents.
