Midterm:
- I used a determinant of health (DoH) framework to assess patient’s health.
(eg. Engaging my patient in discussion about stressors and practicing writing out a genogram).
- Exploring objective and subjective data gathered from patients, medical chart, and primary nurse, I was able to identify which DoH are directly and indirectly impacting patient’s health.

- I was able to identify health patterns, barriers (language) and capacities (positive thoughts and hopeful about future), and health promotion practices (healthy eating, physical activity) with the patient through exploration of subjective and objective information.
- I was able to recognize the influence of culture, and language on health status. (E.g., Both of my patient struggle a bit with English, so were unable to express their concerns.
One of my patients follows Buddhism and other Sikhism. They tied religious beliefs with positive outcomes for mental health. They have positive thinking that things will get better, and always pray, and follow their religion).

- I developed goals at the beginning of this clinical and identified strategy to meet those goals.
- I developed care plan and evaluate this daily.
- Reflecting on my practice, I was able to deal with the emotional situation being reliable and a non-judgmental listener. I felt that my ability to manage and reflect upon my own emotions became stronger with my practice.
- It has provided me with the opportunity to work with groups of individuals in order to achieve positive change.
- I realized that being there for the patient, active listening to what they have to say, and showing empathy helps to de-escalate anxiety level.
