Anthony is physically healthy. Anthony takes medication for seasonal allergies. Anthony health is stable. Anthony receives routine care and medication management for Attention deficit hyperactivity disorder (ADHD). There were no reports of Anthony having developmental issues. Anthony display emotional and behavioral issues at school. Anthony has reports of verbal and physical, aggressive behaviors. Anthony will receive individual therapy.
Per referring agency, Anthony does display age appropriate behaviors. Anthony struggles with peer interaction at school and community. Anthony had a school suspension before SAFY placement.
The referring agency will schedule and supervise all face to face visits with the maternal grandmother. Anthony is allowed to have supervised phone calls with the maternal grandmother.
The referring agency will collaborate with the foster parent to schedule an appointment for individual therapy.
It is recommended Anthony receive daily (PRS), weekly Family Support (FS) medication management of the MUSC foster care support clinic, and individual therapy. Anthony needs an IEP review for educational accommodations.
