Inquiries and serious case reviews are required when a child dies because of neglect,suspected or actual abuse,In England the reviews are conducted by the local safeguarding children's board when serious case reviews happen they bring together all the organisations that are involved in the case these organisations will then do their own investigation into their management of the case and make recommendations that will be used in future improvements. Serious case reviews can also be used when a child dies in custody, Sustains a life threatening injury through abuse or neglect, if they have been subject to serious sexual abuse or a child dies from suspected suicide or if the case reviews gives rise to concerns about inter-agency working to protect
