Originate: at the pubis

Inserts: along the medial edge of the femur, except for the gracilis, which inserts at the top, medial aspect of the tibia.
Tibialis anterior
Originates: at the proximal 2/3 of lateral side of tibia

Inserts: at the foot near the first metatarsal.
Biceps Brachii
Originates: the long head at the superior margin of the glenoid fossa (the socket) of the scapula, and the short head at the coracoid process (top) of scapula (crossing the shoulder joint)

Inserts: at the tuberosity (protuberance) of the radius and fascia (connective tissue) of forearm.
Brachialis and brachioradialis
Originates: lower front of humerus

Inserts: at top of the ulna and the brachioradialis at the distal end of the radius.
Coracobrachialis
Originates: at the scapula, at the tip of coracoid process

Inserts: middle third of medial surface of the humerus
Triceps brachii
Originates: on the scapula
below the glenoid fossa; the lateral and medial heads
both originate on the posterior part of the humerus.

Inserts: olecranon process of the ulna (the olecranon is the point of the elbow)
Trapezius
Originates: At the cervical and thoracic vertebrae and at the base of skull

Inserts: at the clavicle and scapula
Latissimus dorsi
Originates: at the lower six thoracic, the lumbar and the sacral vertebrae, the crest of the ilium and sacrum, and the posterior surface of lower four ribs

Inserts: upper, medial portion of the humerus.
Erector spinae
Originates: aponeurosis, a strong connective tissue at the posterior iliac crest and sacrum

Inserts: in to all the vertebrae and ribs, and some at the base of the skull.
Gluteus maximus
Originates: at the posterior quarter of the iliac crest and sacrum and the posterior surface of the lower sacrum and coccyx;

Inserts: at the femur and the iliotibial band
The gluteus medius and minimus
Originate: at the crest and
outer surface of the ilium

Insert: at the anterior, superior portion of the
femur.
hamstring muscles
Attach to the ischium (your “sit” bone); the biceps femoris

Inserts: at the lateral side of the head of the fibula,
and the semimembranosus and semitendinosus attach
to the tibia.
Gastrocnemius
Originates: at the posterior surfaces of the femur’s inferior heads above the knee

Inserts: at the calcaneus (heel bone) via attachment to the Achilles tendon.
Soleus
Originates: at the proximal two-thirds of the posterior surfaces of the tibia and fibula (near the top)

Inserts: at the calcaneus via attachment to the Achilles tendon.
Peroneus Longus
Originates: at the upper two-thirds of the fibula

Inserts: in the foot at the base of the of first metatarsal.
