ASSESSMENT |
- Review orders for timing, frequency, and if patient has anything conditions that needs to be alerted -2 IDENTIFIERS -Check patients understanding of procedure, the purpose and importance of BGM. |

-Antiseptic device -Lancet device - Sterile Lancet -BG meter -BG strips -Clean gloves - Cotton ball/paper towel |

IMPLEMENTATION - BEFORE |
-Get patient to wash hands
-Make sure patient is in a comfy position (semi-fowlers) -Make sure strips are for right device -Assess skin (ask for perference) |

IMPLEMENTATION - PROCEDURE
|
- APPLY GLOVES, remove cap/insert lancet into device - Wipe puncture site - Puncture site with lancet - Apply blood to strip (slanted downwards) |

IMPLEMENTATION - AFTER
|
- Apply cotton ball to site, get patient to apply pressure
- Discard strip/lancet into bio-hazard receptacle -Remove gloves and WASH HANDS |

EVALUATION
|
- Observe site for any bruising or prolonged bleeding
- Evaluate glucose level. Hypoglycemic? Hyperglycemic? What would you do? |

What would you do if the patients hands were too cold to get a sample? - Lack of circulation. |
Get patient to rub hands together to increase circulation. |

Puncture site continues to bleed. |
-Apply pressure to wound.
-Raise hands above head to slow down circulation to the site. -If bleeding continues, contact the HCP. |

The meter malfunctions. |
Repeat test. New everything. |

Hyperglycemia or hypoglycemia |
- Continuous monitoring.- Follow facility policy. - Check medical record for any medications that need to administer if BG is too high/low. If not, notify HCP.- Administer insulin or carbohydrate source as ordered (depending on level)- Notify HCP of patients response.
