Six-year-old Melissa is suffering from diarrhea, steatorrhea, impaired absorption of macronutrients. She has experienced an extreme weight loss since her last visit. Her mother reports that Melissa often gets these weird, itchy, blistering skin rashes after she eats. In order to diagnose Melissa’s problem a small bowel biopsy was performed and it revealed blunting of the villi (copies of the histology images are attached for reference). An ELISA test was performed on blood from Melissa and the ELISA revealed elevated levels of anti-tissue transglutaminase and immunoglobulin A (IgA). Additional information obtained during the physical exam revealed that Melissa is below the 5th percentile for her age group in weight, despite the fact that her two sisters are well above the 50th percentile for their respective age group.
