% lower 1st molar to show distal canal |
30% |
Enigma to endodontist |
Mandibular 1st premolar Bifurcations and trifurcations are very common |
Variations seen in pulp canal |
Occurrence of more than 1 root |
Mandi 4> maxi 4> mandi 3> maxi 3 |
Accessory canals are common in |
Apical 1/3 of root |
Pulp anatomy of Mandi 1st molar |
1. No. Of roots = 2
2. No. Of canals = 3 (mesiobuccal , mesiolingual , distal)
3. Mesiobuccal orifice- under mesiobuccal cusp Explored- mesiobuccoapical inclination
4. Mesiolingual orifice- below central groove. Explored- distobuccal direction
5. Distal orifice- buccolingual centre of the tooth. Explored- mesial direction. 6. Access opening- usually trapezoidal Maybe rectangular |
Maxi 1st molar |
1. 3 roots 2. 3 canals (rarely 4)- palatal, mesiobuccal, distobuccal |
Perforations |
Mandibular 1 > maxillary 4(mesiodistally narrow) |
Incidence of 3rd root in upper first premolar |
6% |
Incidence of distal root with 2 canals in mandibular molar |
30% |
Dens in dente in maxi 2 |
0.04%-10% |
MB2 prevalence |
95.2% |
2 canals in mandibular 1 |
20%-41.4% |
Access preparation in mandi 4 |
Lingually held bur Floor is NOT kept perpendicular to long axis of tooth. |
Bifurcation in single rooted teeth |
Mandi 4> mandi 3 > mandi 1>= mandi 2> maxi 4,3 Least common in maxi 1,2 |
C- shaped canals |
Fusion of mesial and distal root in either lingual or buccal region. Category 1- continuous c shaped canal Category 2- semicolon shaped Category 3 - C shaped orifice with distinct canals Mandi 7 >6> 8> maxi 7>6>8>mandi45> maxi 2 |
How to detect undetected canals |
1. Dark line parallel to file- Follow the image of test file in the length of the tooth film , particularly in coronal part of root, if an extra black line running parallel to the instrument - suggestive of additional canal. 2. Fast break - If there is sudden change in radiolucency within canal |
